Job Description:
Description Experienced and highly trained pharmacist who works
in a specialized area performing high level functions (e.g.,
Oncology/Infusion Center, Pediatrics/Neonates, Decentralized Roles
e.g., Unit Based Emergency Department Pharmacist) Works in 1 or
more specialty areas greater than 50% of the time Specialty areas
include but not limited to: Oncology/Infusion Center,
Pediatrics/Neonates, Decentralized Roles (e.g., unit-based ED
pharmacist) Proactively contributes to the decision-making process
and the development of patient-centered care plans Deliver
satisfactory and valued services and finding mechanisms to reduce
medication costs Participate in quality, safety and other project
work related to the specialized area Assists in the development of
policy and procedures Unit Based Obtains a patient’s medication
history using multiple sources of validation Provides patient
education using teach back method Conducts antimicrobial
stewardship activities Reviews treatment appropriateness of urinary
tract infections Attends codes and bedside medication emergencies
Rounds with the team, if applicable Job Function: 01. Profiling /
Transcribing: Reviews patient profiles to assure appropriate
therapy based on patient specific information that includes at
least the following: age, gender, diagnosis, co-morbidities,
relevant laboratory values, allergies, height, weight and BSA,
pregnancy and lactation status. Reviews and transcribes all
medication orders including specialized orders based on approved
protocols accurately and timely verifying therapy, correct
medication, route, dose and frequency. Makes recommendations for
alternate treatments when necessary. Records relevant information
and drug waste in EHR. Job Function: 02. Dispensing Medications:
Dispenses medications, prescriptions, miscellaneous items
accurately and promptly. Processes requests via phone, window, fax,
and computer in a professional and timely manner. Obtains
medications when needed to provide optimal patient care using
economical or alternative options and taking into consideration
patient admission status (inpatient vs. outpatient). Legibly signs
all paperwork and/or labels associated with these processes. Job
Function: 03. Checking: Checks unit dose accurately making
corrections to patient profiles if necessary. Checks medications
before being placed in automated dispensing machines and floor
stock to assure accurate fill. Checks IVs, batching, patient own
medications, repackaging and extemporaneous preparations for
accuracy. Checks crash cart trays, rapid response boxes, and
miscellaneous emergency kits. Legibly signs all paperwork and/or
labels associated with these processes. Job Function: 04. Sterile
Preparations: Prepares, supervises and ensures aseptic compounding
of IVs including antineoplastic agents correctly and in a timely
manner promoting efficiency and cost effectiveness. Reviews levels
of batched IV medications, batches, supplies to avoid waste,
documents all required information, and assures batches are checked
and put away promptly. Demonstrates a strong knowledge base of IV
equipment, sterile technique, total parenteral nutrition, and safe
handling of hazardous drug preparation and precautions. Legibly
signs all paperwork and/or labels associated with these processes.
Job Function: 05. Clinical: Participates in clinical initiatives.
Initiates appropriate clinical interventions and clarifications
with corresponding documentation. Demonstrates compliance with
Pharmacy and Therapeutics Committee initiatives. Reviews pertinent
information to monitor patients’ therapies and makes appropriate
recommendations eliminating duplicate, unwarranted, or
inappropriate therapy. Converts non-formulary medication requests
to appropriate formulary alternatives whenever possible. Job
Function: 06. Drug Information: Provides drug information to
physicians, nurses, other health care professionals, and
patients/family members. Demonstrates and shares knowledge of
various drug information resources and knows how to research and
interpret data and information. Qualifications Education/Experience
Graduation from an accredited school of pharmacy required Current
NJ Board of Pharmacy license required 5 years working in a
specialized role (Oncology/Infusion Center, Pediatrics/Neonates,
Decentralized Roles (e.g., unit-based ED pharmacist) Doctor of
Pharmacy (PharmD) required PGY-1 Pharmacy Residency preferred Board
Certified in the specialized area preferred Nationally recognized
certifications in the specialized area preferred
