Nurse Practitioner | Family Practice Location: New Windsor, NY Employer: Lancesoft Inc Pay: Competitive weekly pay (inquire for details) Start Date: ASAP About the Position Position Overview We are seeking a Primary Care Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join a collaborative, team-based primary care practice in New York . This is a permanent, outpatient-only role focused on prevention, wellness, acute care, and chronic disease management for an established adult patient population. The APP will function as a licensed, independent provider within New York State regulations and organizational protocols. This role is exclusively primary care and does not include specialty or surgical services . Compensation Base Salary: $120,000 – $130,000 annually Productivity & Quality Bonus: Up to $5,000 annually Schedule Flexibility: Choice of 4-day or 5-day workweek Strong emphasis on work-life balance Primary Responsibilities Clinical Care Deliver comprehensive primary care, including preventive services, annual wellness visits, acute illness management, and chronic disease care. Perform complete and focused histories and physical examinations using evidence-based guidelines. Diagnose and manage common primary care conditions; coordinate care for complex patients in collaboration with physicians and the care team. Prescribe medications, therapies, and treatments within scope of practice. Order, interpret, and follow up on labs, imaging, and diagnostic studies. Develop and monitor individualized care plans and treatment goals. Emphasize preventive care, lifestyle counseling, and patient education. Patient Communication & Care Coordination Educate patients and families on diagnoses, treatment plans, medications, and preventive strategies. Collaborate closely with physicians, nurses, medical assistants, and care coordinators. Participate in daily huddles and interdisciplinary care planning. Coordinate referrals and follow-up care as needed. Documentation & Quality Document all encounters accurately in the electronic medical record (EMR). Utilize population health tools to support quality metrics and chronic disease outcomes. Ensure compliance with clinical guidelines and billing documentation standards. Professional Practice Maintain up-to-date knowledge of primary care best practices. Participate in continuing medical education (CME) and professional development. Engage in provider meetings, trainings, and quality improvement initiatives. Practice within a patient-centered medical home and team-based care model.
