OverviewCommunity Health Center (CHC) is hiring!Family Nurse Practitioner, Primary Care In KeyportCommunity Health Center (CHC) offers comprehensive, community based, preventive, primary care and wellness services for New Jersey residents of all ages at four convenient locations.CHC health care services are designed to meet the specific needs of the entire family - from annual check-ups and immunizations to sick visits, mental health counseling and critical prenatal care.We are currently looking for a passionate Nurse Practitioner to join our Primary Care Center team. In this full-time role, you will be responsible for providing holistic primary care services in collaboration with our Community Health Center team in Keyport. You will help improve patient health outcomes and support the overall well being of the community. • Supports organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
• Provides direct family and patient centered services to primary care patients at the sites within established productivity guidelines
• Maintains complete patient records using the Electronic Medical record and complies with all reporting mechanisms
• Ensures compliance with the New Jersey Department of Health Ambulatory Care Regulations for Licensure as well as requirements of other licensing and accrediting bodies, health maintenance organizations and other insurance carriers
• Collaborates in team based care in the Patient Centered Medical Home model
• Participates in Peer Review and other Quality Improvement activities
• Collaborates in the provision of in-service education for staff
• Adheres to CHC infection control and safety policies, and guidelines of regulatory agencies
• Participates in community outreach programs regarding CHC services and programs
• Supervises staff in conjunction with the organization's managerial and leadership practices

  • Licensure as an Advanced Practice Nurse in NJ
  • Master's degree in Nursing from an NLN accredited program
  • Certified as a Family Nurse Practitioner with prescriptive authority; maintain a CDS and DEA
  • 2+ years of primary or ambulatory care advanced practice nursing experience preferred; pediatric experience preferred.
  • Bilingual (Spanish/English) preferred
  • Car and valid driver's license Working Conditions/Physical Demand : Working indoors. Exposure to blood, body fluids and communicable diseases. Sitting, fine motor hand movement, standing and walking.
